Tying identity information together?

Getting data from multiple sources is great but how in practice is the "digital identity" aspect handled; how do you know the user MrDoe on forum X is MrSmith on forum Y, and user MrJones on Drupal CMS system Z etc? I mean how can you tie these identities together? Ideas on a postcard welcome :)

It's a interesting question: how to know if several Users accounts are
the same Person. In SWAML we have an old task [1] about this, because
each Person could use his several mail address to send messages to a
mailing list.

I know clearly the way (using his FOAF profile: find it and link all the
Users related with that Person), but not how. For the moment we have
hardcoded the first part, because I don't know a service that gives the
URI of a Person from a mail address.

What's your opinion?
